INSTRUCTIONS:
- Complete this "2008 CDC Alumni Services Registration Form (PDF)"
- Prepare a check made payable to "CSUEB CDC" for the appropriate amount
- Complete the attached "NET I.D. Activation Form (PDF)" if you do not possess a Net I.D.
- Return this "2008 Alumni Services Registration Form (PDF)," "Net I.D. Activation Form (PDF)" (if applicable), and check to either CDC location
